Of everything I’ve tested in 18+ years of studying cognitive performance, the physiological sigh is the intervention with the best ratio of effect to effort. It costs nothing, requires no equipment, takes about ten seconds, and can be done in a meeting without anyone noticing. It won’t transform your life — I’ll be straight with you about the size of the effect — but as a tool for pulling yourself down from acute stress before it wrecks your ability to think, nothing else I’ve tried is as fast or as reliable.

What the physiological sigh actually is

You already do this, roughly once every few minutes, without noticing. A physiological sigh is a double inhale followed by an extended exhale: a full breath in through the nose, then a second shorter sip of air on top of it, then a long, slow release through the mouth. Your body performs this automatically during sleep and spontaneously when you’re upset — the shuddering breath of someone who has been crying is the same pattern.

The reason it exists is mechanical. The lungs contain millions of tiny air sacs called alveoli, and over time some of them collapse, making gas exchange less efficient and allowing carbon dioxide to accumulate. That second, sharper inhale pops the collapsed sacs back open, and the long exhale then offloads a large volume of carbon dioxide at once. What makes it interesting for us is that you can do it deliberately — taking a reflex your body already uses to reset itself and deploying it on demand, exactly when stress is climbing.

Why a long exhale calms you down

The key principle in all calming breathwork is the ratio between inhale and exhale, and it rests on a genuine physiological asymmetry: inhaling tends to speed the heart slightly, while exhaling tends to slow it. Make your exhales consistently longer than your inhales and you tilt the autonomic nervous system toward its parasympathetic — “rest and digest” — branch. A systematic review of slow-breathing research found that techniques under about ten breaths per minute produce consistent shifts in autonomic balance, increased heart rate variability, and measurable changes in brain activity alongside reports of reduced arousal and improved wellbeing (Zaccaro et al., 2018).

This is also why breathwork is worth taking seriously as a cognitive tool rather than just a relaxation one. The prefrontal cortex — the region running focused, goal-directed thinking — is unusually vulnerable to stress, and even mild uncontrollable stress rapidly impairs its function (Arnsten, 2009). A technique that reliably lowers acute arousal in under a minute is, functionally, a technique for protecting your capacity to think clearly under pressure. That connection to HRV and stress optimisation is where breathwork earns its place in a serious protocol.

What the controlled trials show

The most useful study here is a randomised controlled trial run remotely over a month, comparing three breathing practices against mindfulness meditation, each done for just five minutes a day. Participants were assigned to cyclic sighing (prolonged exhales), box breathing (equal inhale, hold, exhale, hold), or cyclic hyperventilation with retention. The result: breathwork — and especially the exhale-focused cyclic sighing — improved mood and reduced respiratory rate more than mindfulness meditation, with benefits growing the more consistently people practised (Balban et al., 2023).

Two details in that finding are worth pausing on. First, the winning technique was the gentlest one — not the dramatic hyperventilation protocol that gets the most attention online. Second, five minutes a day was enough to produce measurable change, which is a genuinely low bar for a daily practice.

Now the honest calibration. When researchers pooled the wider literature — 12 randomised controlled trials covering 785 adults — breathwork produced a statistically significant but small-to-medium reduction in self-reported stress (Hedges’ g = −0.35), and most of the included studies carried a moderate risk of bias (Fincham et al., 2023). So the direction of the evidence is clear and positive, but the magnitude is modest and the literature isn’t yet strong. Breathwork is a real, useful tool — not a cure for chronic stress or anxiety, and not a substitute for professional help if you need it.

The techniques worth knowing

Three practices cover almost every situation. The single physiological sigh is your emergency brake: two inhales through the nose, one long exhale through the mouth, repeated once or twice. Use it in the moment — before a difficult conversation, when frustration spikes, when you notice your shoulders climbing. It works within seconds and is completely invisible to anyone watching.

The five-minute cyclic sighing practice is the daily version tested in the trial: the same double-inhale, long-exhale pattern repeated continuously for five minutes, ideally at a consistent time each day. This is the one with the best evidence for cumulative mood benefit. And box breathing — four counts in, four hold, four out, four hold — is the steadier option many people prefer for pre-task settling; it performed well in the trial too, just not as strongly as cyclic sighing.

A note on what I’ve deliberately left out. Intense hyperventilation-with-retention protocols are popular and were included in the trial, but they carry genuine risk — repeated breath-holding can cause fainting, and doing that in water or while standing has caused serious injuries and deaths. They also performed no better than the gentle techniques for mood. Given equal or lesser benefit and vastly greater risk, I don’t recommend them for the purpose this article is about. If you’re drawn to more demanding physiological stressors, deliberate cold exposure has a better-characterised risk profile and is easier to do safely.

The physiological sigh is the only technique in my entire toolkit where I can feel the effect in real time. Most of what I test — supplements especially — requires weeks of tracking to detect anything at all. This one produces a noticeable drop in physical tension within about two breaths, which is why it survived in my routine when far more elaborate protocols didn’t. I use it before difficult conversations and whenever I catch my jaw tightening at the desk.

Where I’ve been less consistent is the five-minute daily practice, and I want to be honest about that rather than pretend otherwise. I’ve run it in stretches and my Oura HRV trends looked mildly better during those periods, but I can’t disentangle that from everything else going on in those weeks — sleep, training load, work stress. It’s suggestive at best, and I’d treat my own data here as weak. The acute effect I’m confident about; the cumulative effect I’m taking mostly on the trial’s evidence rather than mine.
